Delivery of courses

You must prove that you have delivered at least one course in the last 12 months. A copy of an award sheet is appropriate.

What you will need to complete this form

1. A copy of your current RLSSWA resuscitation award.

2. A copy of your current Working With Children Check.

3. Details regarding two professional development sessions you have completed in the last year.

4. Proof that you have trained and assessed at least one aquatic course in the last year. A copy of an award sheet is appropriate.

5. A signed copy of the Third Party Agreement for individuals.
